Provisions of Program:
- One time payment of the annual fee. No monthly installments
- No refunds on this plan. Not transferable to other offices
- Services and fees are guaranteed for one year
- Initial fee is non-refundable
- Our program is for you and is non-assignable to another person
- One year time limit for completion of all services provided by the program
- Work started before the end of the Program year will be covered
- It is responsibility of the patient to make sure they receive and show up for appointments
- Appointments are required, except for emergency visits. There is a fee for a cancelled appointment without giving advance 24 hour notice
- Only dental procedures listed in the Fee schedule are included
- Referred specialty (By Specialist) work is not included in this plan
- Payment is due at the time service is rendered
